St. Croix Hospice is seeking a Hospice Aide (CNA) to join our in-home care team in Wisconsin. Under RN supervision, you will provide compassionate, hands-on personal care to patients and support daily activities to promote comfort and dignity at the end of life.

You will travel to patient homes, document services, and assist with meals while maintaining patient safety and quality of life. A valid CNA certification and driver's license are required for this full-time role.

Qualificações

• CNA certification in the state of practice is required.
• A valid driver’s license and clean driving record are required.
• Record vital signs and report observations to the care team.
• Provide personal care and assist with meals as needed.

Responsabilidades

• Provide personal care including bathing, skincare, oral hygiene, and dressing.
• Record and communicate vital signs and changes to the care team.
• Assist with meal planning, preparation, and feeding.
• Travel to patient homes throughout the day to provide care as scheduled.
• Maintain accurate documentation of services provided.
