St. Croix Hospice in Faribault, MN is seeking a Hospice Aide (CNA) to join the interdisciplinary care team. Under RN supervision, you provide direct personal care to hospice patients and help implement each patient’s Plan of Care.

This full-time, travel-intensive role requires an active CNA license, eligibility for a background check and drug test, and a valid driver's license. You’ll support patients and families in their home setting with compassion and respect.

Formación

• Active Certified Nursing Assistant (CNA) license in the state of practice.
• Must be at least 18 years of age.
• Ability to pass background study, consumer report and drug test.
• Valid State Driver’s License and access to an automobile, with acceptable driving record.

Responsabilidades

• Provide personal care including bathing, dressing, oral hygiene, and bed linen changes.
• Assist with mobility, transfers, and exercises as instructed by nursing staff.
• Assist with meals, feeding, and light household tasks as assigned.
• Monitor and report patient condition changes to the RN Case Manager.
• Administer routine skin care to prevent breakdown and ensure patient comfort.
• Support medication reminders per state regulations.
• Maintain a clean, safe patient environment.
• Provide emotional support for patients and respite for family caregivers.
• Use equipment safely for patient care needs.
• Adhere to documentation and HIPAA standards.
• Participate in quality assessment and performance improvement activities.
