Job summary

St. Croix Hospice seeks a compassionate Hospice Aide (CNA) to join our team in a home-based care setting.

You will work under the supervision of an RN, providing direct personal care, mobility support, and medication reminders as per state regulations, while documenting care and communicating changes to the RN Case Manager. The role requires an active CNA license, being at least 18, and the ability to pass background checks and a drug screen.

Qualifications

• Active CNA license in the state of practice.
• Must be at least 18 years old.
• Ability to pass background, consumer report, and drug test.
• Valid driver’s license and access to reliable transportation.
• Ability to lift/push/pull up to 50 lbs.
• Comfort with home-based patient care and documentation.

Responsibilities

• Provide direct personal care (bathing, dressing, hygiene) and support with daily activities.
• Assist with mobility, transfers, and exercises as directed by nursing staff.
• Support meal preparation and light housekeeping as assigned.
• Monitor patient condition and report changes to RN Case Manager.
• Maintain patient environment safety and HIPAA-compliant documentation.
• Provide emotional support to patients and respite to family caregivers.
