Providence PACE Seattle is seeking a field-based care manager to ensure home care services meet PACE and state regulations while supporting participant independence. The role travels to the South Seattle area and beyond to coordinate, assess, and supervise services in participants’ homes.

You will work with contractors and home care agencies to align care with program goals, document care plans, and promote satisfaction and retention.

Formación

• 2–4 years coordinating, managing, monitoring, and providing community social/health services.
• 2 years administrative experience in healthcare setting.
• Driving is required; provide valid driver's license and auto insurance.

Responsabilidades

• Assure home care services comply with PACE and state regulations.
• Organize, coordinate, direct and evaluate care functions.
• Provide oversight of services in participants' homes to promote independence.
• Assess, coordinate, consult and supervise home care services.
• Liaise with contractors such as home care agencies and individual providers.
