Job summary

Providence PACE Seattle-Seattle seeks a Home Care Coordinator to organize, coordinate, and supervise home care services for participants in the South Seattle area and beyond. You will ensure services comply with PACE and state regulations while promoting participant satisfaction and independence.

Responsibilities include oversight of care in participants' homes, liaising with contractors, and coordinating assessment and supervision activities to maintain high-quality care.

Qualifications

• 2–4 years of coordinating, managing, monitoring, and providing community social/health services.
• 2 years of administrative experience in a healthcare setting.
• Must possess a valid driver license and auto insurance.

Responsibilities

• Organize, coordinate, direct and evaluate care functions.
• Provide oversight of home care services in participants' homes to promote independence.
• Assess, coordinate, consult and supervise home care services.
• Act as liaison with contractors such as home care agencies and individual providers.
• Travel to support South Seattle area and other areas as needed.
