Job summary

PacificSource PACE LLC in Oregon seeks an experienced Home Care Supervisor to oversee in-home care for older adults in the PACE program. You will develop and manage care plans, lead the interdisciplinary team, and ensure coordinated, high‑quality services that support health, independence, and dignity.

You will perform in‑home assessments, supervise aides, document progress, coordinate with families and providers, and participate in committees as required.

Qualifications

• Minimum 2 years of experience in case management, care coordination, and/or home and community-based services.
• Preferred: experience with geriatric or frail populations.
• Valid, active Oregon license in health field required (RN, OT, PT, LCSW, PTA, COTA).
• CPR certification (BLS or ACLS) required.
• Valid driver’s license required.

Responsibilities

• Manage delivery of home care services for PACE participants.
• Coordinate participant care plans for medical, social, and personal needs.
• Provide in-home case management and supervise home-care aides.
• Ensure authorized service delivery, timeliness, and positive participant experience.
• Maintain accurate records of activities and changes in participant conditions.
• Support participation in interdisciplinary team planning and meetings.
