Job summary

PACE Southeast Michigan seeks a Home Care Services Manager to oversee the Home Care team and coordinate nursing assistant activities in our PACE program. You will facilitate interdisciplinary planning and complex care meetings to ensure standardized care and proper service authorization.

You will collaborate with the Center Manager, lead staff, and the Medical Director on clinical operations, budget adherence, quality initiatives, and performance metrics while maintaining CMS-compliant practices.

Qualifications

• Bachelor degree required; BSN preferred.
• Three years of clinical long-term care experience, including one year with frail elderly.
• Two years of supervisory experience.

Responsibilities

• Supervises and coordinates Home Care Activities at PACE Southeast Michigan.
• Facilitates IDT and care plan meetings to ensure consistency and authorization for participants.
• Assists Medical Director on clinical and operational issues and special projects.
