Job summary

PACE seeks a Home Care Coordinator RN to supervise staff and coordinate scheduling and documentation of the care plan for personal and non-skilled home care, at both the PACE Center and participants' homes. The role emphasizes teaching, counseling, and preventive/restorative services to promote health and independence among participants.

The RN will assess needs, contribute to care plans, implement care plans, re-evaluate, and ensure compliance with PACE Program standards.

Qualifications

• Must be a Registered Nurse with an active license.
• Experience coordinating home-care services and supervising staff.
• Strong communication and documentation skills.
• Ability to teach, counsel, and support preventive/restorative care.

Responsibilities

• Manage staff roster and schedules for home care to optimize time and cost.
• Train and supervise Center-based and in-home caregiver staff.
• Communicate with participants and families about home care needs and coverage.
• Maintain accurate home care records, billing, and documentation.
