Job summary

Responsibilities

• Conduct comprehensive assessments and monitor residents.
• Administer medications and manage IV therapy per orders.
• Perform wound care and skin integrity assessments.
• Update care plans for acute changes in condition.
• Collaborate with physicians, therapists, and social workers for coordinated care.
• Document all care in the facility's EHR accurately.
• Educate residents and families about care plans and safety.
