Job summary

HealthForce Healthcare is seeking a Travel Registered Nurse (RN) specialized in Long-Term Care for a contract position in Dubuque, IA. You will deliver high-quality nursing care to residents in a skilled nursing environment, managing chronic condition care plans and daily wellness while coordinating with an interdisciplinary team.

The role requires LTC/SNF experience, BLS certification, and an Iowa RN license (or compact state).

Qualifications

• Experience in Long-Term Care (LTC) or Skilled Nursing Facility (SNF) settings.
• Proven ability in chronic disease management and geriatric nursing.
• Proficiency with electronic health records (EHR) and care coordination tools.

Responsibilities

• Conduct comprehensive assessments and monitor residents for health changes.
• Administer medications and manage IV therapy as ordered.
• Perform wound care and skin integrity assessments; prevent pressure ulcers.
• Identify acute changes, update care plans and escalate as needed.
• Collaborate with physicians and therapists for interdisciplinary care.
• Document all care and resident responses in the EHR.
• Educate residents and families about care plans and safety.
