Job summary

American Traveler is seeking a Home Health RN for a 13-week assignment caring for a geriatric patient population, requiring WA or compact RN license and 2+ years of experience. This travel role emphasizes skilled nursing in patients' homes with daily 8:00 AM to 4:30 PM shifts and on-call duties.

Qualifications

• Active WA or compact RN license required.
• BLS certification required.
• Minimum 2 years RN experience.
• Two professional references from the past 2 years, at least one from a supervisor.
• SSN and DOB required at time of consideration.
• Proof of WA license application if license is not yet in hand.
• No more than one DUI permitted for consideration.
• On-call responsibilities required.
• May be required to float within the health system.
• Epic EMR experience preferred but not required.

Responsibilities

• Home health setting providing direct skilled nursing care to patients in their homes.
• Geriatric patient population with HF, COPD, wounds, diabetes, post-surgical care, respiratory illness, sepsis, dementia, s/p CVA, and s/p MI.
• Average daily census of 650 patients.
• Day shift schedule, 5x8 hours, 8:00 AM to 4:30 PM.
• On-call responsibilities required.
• Documentation duties include admission assessments, recertification, resumption of care, and OASIS reporting.
• RTO disclosure and up to 7 days for regular offers.
• May be required to float within the health system as operationally necessary.

• Perform initial evaluations of patients and homes to determine safety and eligibility for home health services
• Provide skilled nursing care and education/instruction to patients and caregivers according to the plan of treatment
• Utilize the nursing process of assessment, problem identification, planning, intervention, and outcome evaluation
• Responsibilities may include wound care, medication management, catheter care, tube management, and other skilled nursing interventions
• Documentation duties include admission assessments, recertification, resumption of care, and OASIS reporting
• First-time travelers are welcome to apply
• Local candidates are not accepted; must reside outside a 60-mile radius of the facility
• Previous PeaceHealth employees must be separated from the health system for at least 1 year before returning as a traveler
• RTO must be disclosed and approved prior to contract signing, with up to 7 RTO days for regular offers
• Start date may be delayed up to 8 weeks if WA license is still in process at time of consideration
• Candidates must disclose full vaccination status and Epic experience details
