Job summary

WelbeHealth is seeking a Travel Registered Nurse Specialist to guide and coordinate nursing care across markets in a value-based PACE program. You will provide direct nursing care, conduct regular assessments, educate participants and families, and collaborate with PCPs and the IDT to ensure high-quality outcomes.

The role requires travel up to 100% with multi-state support, flexible scheduling, and CPR/First Aid certification within 180 days.

Qualifications

• Graduate of an accredited school of nursing.
• Unencumbered RN license.
• Minimum of two years nursing experience with frail/elderly population.
• Ability to travel up to 100% and multi-state support as directed.
• Willingness to work varied schedules including evenings/nights/weekends.
• CPR and First Aid certification within 180 days of hire (preferred at hire).

Responsibilities

• Provide nursing care to participants as assigned.
• Conduct PACE required nursing assessments (minimally every 6 months).
• Develop a discipline-specific plan of care with IDT and participants.
• Coordinate care for high-complexity participants across hospital/ER/skilled facilities.
• Serve as triage point to maximize ER diversion and clinic management.
• Dispense medications and reinforce education to participants.
• Coordinate follow-up on patient care measures with other health care personnel.
