American Traveler, LLC is seeking an RN for a PACE Home Care position serving the geriatric population in Battle Creek, MI, requiring an active Michigan RN license. The role involves direct nursing care, wound care, immunizations, and coordination within an interdisciplinary team; Monday–Friday, 8:00 am–4:30 pm, with travel between centers and occasional on-call.

The successful candidate will provide high-quality home-based care, participate in care planning with PCPs, and ensure documentation

Qualifications

• Active MI RN license required.
• Associate degree required; BSN preferred.
• Minimum of one year with frail/elderly/long-term care populations.
• Home care and wound care experience preferred.
• Current CPR certification by AHA required, or willingness to obtain.
• Proficiency with Microsoft Word and Outlook required.
• MI driver's license and vehicle insurance required.
• Reliable transportation for travel between centers.
• Immunizations current or medical waiver documented.

Responsabilités

• PACE home care setting for older adults as an alternative to nursing home care.
• Interdisciplinary care team collaboration with physicians, nurses, social workers, and aides.
• Manage geriatric patients with acute and chronic medical conditions.
• Monday–Friday, 8:00 am–4:30 pm schedule.
• Regular home visits for care and treatment per PCP orders.
• On-call responsibilities as needed, including weekends/holidays.
• Provide direct nursing care: wound care, medications, immunizations, labs, assessments.
• Triaging emergencies and handling complex clinical situations independently.
• Collaborate with staff, participants, and families using participant-centered care.
• Document in medical records per policy; may encounter infectious waste.

• Current immunizations required (Hepatitis B, TB, MMR, Varicella, Tdap, Flu, COVID-19) or medical waiver documentation

Responsibilities

• PACE (Program of All-Inclusive Care for the Elderly) home care setting serving older adults as an alternative to nursing home care
• Part of an interdisciplinary care team including physicians, nurse practitioners, social workers, therapists, and aides
• Geriatric patient population with acute and chronic medical conditions
• Monday through Friday schedule, 8:00 am to 4:30 pm
• Includes regular home visits for care and treatment as ordered by the PCP
• On-call responsibilities as needed, including weekends and holidays
• Provide direct nursing care including wound care, medication administration, immunizations, lab draws, and assessments
• Triage emergent situations and manage complex clinical situations independently
• Collaborate with licensed and non-licensed staff, participants, and families using a participant-centered care model
• Document in participant medical records per policy
• May be exposed to infectious waste, diseases, and hazardous materials as part of job duties
• Subject to callback during emergency conditions such as severe weather or disasters
• Must be able to push, pull, move, sit, or stand for long periods and lift up to twenty pounds
• Must be 18 years of age or older
