WelbeHealth, LLC. seeks a compassionate Home Health RN to provide skilled nursing care in participants' homes under CMS guidelines.

You will work with the IDT to develop and carry out individualized care plans, deliver treatments, and educate patients and families to promote healthier, more independent lives. The role emphasizes travel-inclusive hourly pay, flexible scheduling, and a strong focus on work-life balance.

Qualificações

• Graduate of an accredited school of nursing.
• Unencumbered RN license.
• Minimum of one year of nursing experience in a clinical setting with frail or elderly population; home health experience preferred.
• Reliable means of transportation.
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR/First Aid preferred at hire.

Responsabilidades

• Conduct home health nursing assessments to determine equipment needs and patient goals.
• Actively participate in IDT meetings to develop patient care plans.
• Deliver and document home health nursing interventions per care plans, maintain a safe environment, respond to physician orders, administer medications and perform tests/treatments.
• Instruct patients, families, and caregivers on disease processes, self-care techniques, and prevention strategies.
• Track and schedule home health visits in collaboration with Home Health Manager or scheduler.
• Collaborate with physicians and IDT for smooth transitions between settings (hospital, SNF, home).
