Job summary

WelbeHealth in Los Angeles seeks a Travel Registered Nurse Specialist to support the PACE program, providing nursing care to frail seniors, conducting assessments, creating plans of care with the IDT, coordinating transitions, dispensing medications, and ensuring high-quality, value-based care. Must travel up to 100% across multiple markets.

Holds an active RN license with 2+ years of clinical experience in elderly populations; compensation includes base pay and bonus with comprehensive benefits.

Qualifications

• Graduate of an accredited school of nursing.
• Unencumbered RN license.
• Minimum of two years of nursing experience with frail or elderly populations.
• Ability to travel up to 100% across markets.

Responsibilities

• Take ownership of care quality and outcomes for assigned participants.
• Conduct PACE nursing assessments at least every 6 months and educate on conditions.
• Develop care plans with the IDT and ensure high-quality value-based care.
• Coordinate transitions of care and act as triage point for escalations.
• Dispense medications and reinforce patient education.
