Job summary

American Traveler, LLC is seeking an RN for a PACE Home Care position serving the geriatric population in Battle Creek, MI, requiring an active Michigan RN license. The role involves direct nursing care, wound care, immunizations, and coordination within an interdisciplinary team; Monday–Friday, 8:00 am–4:30 pm, with travel between centers and occasional on-call.

Qualifications

• Active MI RN license required.
• Associate degree required; BSN preferred.
• Minimum of one year with frail/elderly/long-term care populations.
• Home care and wound care experience preferred.
• Current CPR certification by AHA required, or willingness to obtain.
• Proficiency with Microsoft Word and Outlook required.
• MI driver's license and vehicle insurance required.
• Reliable transportation for travel between centers.
• Immunizations current or medical waiver documented.

Responsibilities

• PACE home care setting for older adults as an alternative to nursing home care.
• Interdisciplinary care team collaboration with physicians, nurses, social workers, and aides.
• Manage geriatric patients with acute and chronic medical conditions.
• Monday–Friday, 8:00 am–4:30 pm schedule.
• Regular home visits for care and treatment per PCP orders.
• On-call responsibilities as needed, including weekends/holidays.
• Provide direct nursing care: wound care, medications, immunizations, labs, assessments.
• Triaging emergencies and handling complex clinical situations independently.
• Collaborate with staff, participants, and families using participant-centered care.
• Document in medical records per policy; may encounter infectious waste.
