WelbeHealth in Pasadena, CA is seeking a Home Health RN to provide skilled nursing in participants’ homes under CMS guidelines. You will work closely with the IDT to implement care plans, deliver interventions, administer medications, and educate families on disease management and self-care.

Qualifications include an accredited nursing program, an active RN license, and at least 1 year of nursing experience with frail or elderly populations; CPR/First Aid within 180 days of hire is preferred.

Formación

• Graduate of an accredited school of nursing.
• Unencumbered RN license required.
• Minimum of 1 year of nursing experience with frail or elderly populations; home health experience preferred.

Responsabilidades

• Conduct home health nursing assessments to determine participant needs and care plans.
• Actively participate in IDT meetings to develop and update care plans.
• Deliver and document nursing interventions, administer medications, and perform tests as ordered.
• Instruct participants and caregivers on disease management, self-care, and prevention strategies.
• Track scheduling in collaboration with the Home Health Manager or scheduler.
