Job summary
Curana Health is seeking a Traveling HRA Nurse Practitioner to perform comprehensive, non-invasive assessments at patients' residences across a region. This full-time role requires frequent travel and aims for 20–30 assessments weekly, with documentation and patient education on wellness and preventive care.

Qualifications
- Active NP licensure in two or more states.
- Unrestricted licensure with ability to obtain more.
- Willingness to travel extensively.
- Experience with ICD-10 and HCC documentation.
- EMR proficiency.
- Excellent communication skills.
- Former SNP/HRA vendor experience preferred.
- Access to a major airport.
Responsibilities
- Travel to and from patients' residences.
- Perform health risk assessments and physical exams.
- Document assessments accurately and timely.
- Conduct medication reconciliation.
- Educate patients on wellness and preventive care.
- Collaborate with Curana and facility teams.
- Maintain confidentiality and regulatory compliance.
