American Traveler is seeking an experienced RN Case Manager for an inpatient Care Coordinator role in Pasadena, CA. Active CA RN licensure and recent Epic EMR experience are required.

The position involves discharge planning, utilization review, psychosocial assessments, and care coordination in an acute hospital setting. Qualifications

• Active CA RN license with current Nursys verification
• BSN preferred
• Minimum 1 year of travel nursing experience
• Recent and demonstrable Epic EMR experience
• Experienced in discharge planning assessments and utilization review
• Strong psychosocial assessment skills required
• Acute hospital case management experience required
• Proof of vaccinations required
• Valid driver's license showing current, permanent address required
• Two professional references required at supervisor level or above (Charge Nurse references not accepted)

Responsibilities

• Inpatient Care Coordinator role within the Case Management department of an acute hospital setting
• Day shift schedule, 5x8 hours, Monday through Friday from 8:00am to 4:30pm
• 8-week assignment length
• Responsible for conducting InterQual reviews and screening transfers for clinical appropriateness
• Perform discharge planning assessments and utilization review functions
• Conduct psychosocial assessments to support care coordination
• Screen patient transfers for clinical appropriateness using InterQual criteria
• Local candidates residing within 75 miles of the facility are accepted at the local rate
• Maximum RTO of 7 days must be submitted at time of consideration, with no additional RTO accepted afterward
