Job summary

Northwell Health is seeking a registered nurse to perform age-specific plan of care using the nursing process and to collaborate with physicians and health-team members. You will use leadership and clinical judgment to coordinate patient care on the unit.

Responsibilities include assessing physiologic and psychosocial health, developing outcomes and care plans, implementing treatments, documenting care, evaluating outcomes, coordinating discharge, and educating patients and families.

Qualifications

• Graduate from an accredited School of Nursing; ADN required, BSN preferred.
• Current NY State RN license and BLS certification.

Responsibilities

• Assess patient’s physiologic health status.
• Assess patient’s psychosocial health status and care needs.
• Identify patient outcome and establish individualized goals.
• Formulate and prioritize the plan of care; coordinate resources.
• Implement the plan and administer prescribed treatments.
• Evaluate care provided and document outcomes.
• Collaborate with physicians and health team members.
• Provide discharge planning and referrals as appropriate.
• Educate patients and families about care and treatments.
• Perform related duties and ensure safety throughout care.
