• Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.

Responsibilities

• Interviews patients, reviews charts and explains policies; assesses multiple health domains and adjust plan of care.
• Identifies patient coping, cultural needs, and coordinates psychosocial support and discharge planning.
• Develops outcome statements, prioritizes care activities, coordinates supplies and medications, documents and communicates plan of care.
• Executes treatments safely, reassesses patients, and delegates appropriately while maintaining patient safety and comfort.
• Demonstrates leadership in planning, evaluating outcomes, and coordinating with physicians and care team members.
