Job summary

Molina Healthcare seeks a Care Coordinator to support care management and coordination across the care continuum. You will work with a multidisciplinary team to ensure progress toward desired outcomes and contribute to strategies that deliver quality, cost-effective member care.

Responsibilities include assessments, care plan development, and ongoing monitoring. Travel may be required, and strong communication, organization, and knowledge of community resources are essential.

Qualifications

• At least 2 years experience in health care, preferably in care management or related setting.
• License and/or certification as required by state contract or regulation.
• Valid driver's license and reliable transportation for travel requirements.
• Knowledge of community resources and ability to work with diverse populations.
• Strong time-management and prioritization skills.
• Excellent verbal and written communication skills.
• Proficiency with Microsoft Office and related software.

Responsibilities

• Complete member assessments within regulated timelines to determine care coordination eligibility.
• Develop and implement care plans with member, family, and care team.
• Conduct telephonic, in-person, or home visits as needed.
• Monitor and adjust care plans based on progress and goals.
• Maintain ongoing member caseload with proactive outreach.
• Promote integration of services across behavioral health and LTSS.
• Facilitate ICT meetings and multidisciplinary collaboration.
• Address barriers to care and provide guidance to members.
• Collaborate with licensed care managers as needed.
• Travel up to 25–40% based on state requirements.
