Job summary

Pride Health is seeking a licensed Physical Therapist Assistant to provide in-home physical therapy in Frankfort, KY and surrounding counties. The role requires travel to Woodford, Mercer, and Anderson Counties, with a local contract and 14-week duration.

Schedule is Monday–Friday, 8:00 AM–4:30 PM, 40 hours per week, with mileage reimbursement. Pay ranges from $38.00 to $41.00 per hour, based on experience and credentials.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Kentucky PTA license.
• Previous Home Health experience required.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Ability to travel throughout assigned territories.

Responsibilities

• Provide skilled physical therapy treatments per the established plan of care.
• Improve patient strength, balance, mobility, and functional independence.
• Monitor progress and document treatment outcomes accurately.
• Communicate status changes and progress to supervising PT and care team.
• Educate patients/caregivers on home exercise programs and safety.
• Maintain timely clinical documentation compliant with regulations.
• Travel to patients' homes within the coverage area.

Pride Health is hiring a Physical Therapist Assistant (PTA) - Home Health to support our client's healthcare facility in Frankfort, KY.

Schedule: Monday - Friday, 8:00 AM - 4:30 PM | 40 Hours/Week

Location: Frankfort, KY 40601

Float / Coverage Area: Woodford, Mercer, and Anderson Counties, Kentucky

Job Type: Local contract

Contract Length: 14 Weeks

Pay Range: $38.00 to $41.00/hour*

Mileage Reimbursement Rate: $0.52 / mile

*Pay offered is based on experience, expertise, credentialing, and education.

Duties:

• Provide skilled physical therapy treatments in accordance with the established plan of care developed by the Physical Therapist.
• Deliver high-quality patient care focused on improving strength, balance, mobility, endurance, and overall functional independence.
• Monitor patient progress and accurately document treatment outcomes and responses to therapy.
• Communicate patient status changes, concerns, and progress to the supervising Physical Therapist and interdisciplinary care team.
• Educate patients and caregivers on home exercise programs, safety precautions, and techniques to support rehabilitation goals.
• Maintain timely and accurate clinical documentation in compliance with agency, state, and federal regulations.
• Travel throughout the assigned coverage area to provide in-home therapy services.

Requirements:

• Graduate of an accredited Physical Therapist Assistant program.
• Current PTA license in the state of Kentucky.
• Previous Home Health experience required.
• Current CPR/BLS certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to travel throughout assigned territories.
