Job summary

Pride Health is seeking a Physical Therapist (Home Health) to support care in Red Bud, IL and surrounding areas. Schedule is Monday–Friday, 8:00 AM–4:30 PM, with a local contract and a 17+ month term with potential extension.

You will evaluate and treat patients, develop care plans, document in EMR, and coordinate with the care team. Requires active PT license and 1 year home health experience; HCHB/OASIS a plus.

Qualifications

• Active Physical Therapist license is required.
• Minimum 1 year of home health experience; familiarity with HCHB and OASIS is a plus.

Responsibilities

• Perform comprehensive physical therapy evaluations and ongoing reassessments.
• Develop, implement, and adjust individualized care plans to improve mobility and functional independence.
• Deliver skilled interventions including therapeutic exercise, gait training, and home safety education.
• Educate patients and caregivers on home exercise programs, fall prevention, and energy conservation techniques.
• Complete accurate and timely documentation using EMR systems.
• Coordinate care with physicians and interdisciplinary team members.
• Communicate changes in patient condition promptly to the care team.
• Participate in case conferences and care coordination activities.
• Ensure CMS regulation compliance and infection control standards.
• Manage a mobile caseload while meeting productivity and documentation expectations.
• Adapt quickly to new environments and patient populations.

Job Details

• Schedule: Monday-Friday | 8:00 AM-4:30 PM
• Location: Red Bud, IL
• Coverage Area: O'Fallon, Belleville, Waterloo, Red Bud, Chester and surrounding areas
• Contract Length: 17+ Months Contract (Possibility to Extend)
• Job Type: Local contract
• Pay Range: $60.00 - $66.00/hour (W-2) Plus Mileage reimbursement
• Mileage Reimbursement Rate: $0.51/ mile
• *Pay offered is based on experience, expertise, credentialing, and education

Duties

• Perform comprehensive physical therapy evaluations and ongoing reassessments to identify impairments, functional limitations, and safety risks.
• Develop, implement, and adjust individualized care plans to improve mobility, strength, balance, endurance, pain management, and functional independence.
• Deliver skilled interventions including therapeutic exercise, gait training, transfer training, balance activities, stair training, and home safety education.
• Educate patients and caregivers on home exercise programs, fall prevention, assistive device use, and energy conservation techniques.
• Complete accurate and timely documentation (evaluations, visit notes, progress reports, and discharge summaries) using EMR systems.
• Coordinate care with physicians and interdisciplinary team members, including nurses, occupational therapists, speech therapists, and home health aides.
• Communicate changes in patient condition, safety concerns, or barriers to progress promptly to the care team.
• Participate in case conferences, care coordination activities, and team collaboration.
• Ensure compliance with CMS regulations, infection control standards, and agency policies.
• Manage a mobile caseload efficiently while meeting productivity and documentation expectations.
• Adapt quickly to new environments, workflows, and patient populations as a travel clinician.

Requirements

• License(s): Active Physical Therapist license
• Years of experience: At least 1 year of home health experience and experience with HomeCare HomeBase (HCHB) and OASIS is a plus
