Job summary

Matrix Medical Network seeks a qualified Nurse Practitioner to perform Comprehensive Health Assessments in patients' homes, including medical history, medications, and social factors. Visits may last up to one hour and require travel to Union, Maine.

You will collaborate with physicians, case managers, and care teams, order tests, and coordinate follow-up care. We offer a $1,500 sign-on bonus, flexible scheduling, mileage reimbursement, and opportunities for growth through FT roles.

Qualifications

• Must hold NP licensure in state of practice.
• Board certification in a relevant NP specialty preferred.
• BLS/ACLS certification required or eligible.

Responsibilities

• Conduct adult/geriatric health assessments in patients' homes.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Provide patient education and follow-up planning with PCPs.
• Travel to patient homes and SNF facilities as required.
