Job summary
Matrix Medical Network is seeking a Nurse Practitioner to perform Comprehensive Health Assessments in home settings across Maine, with travel to Greene area. Each visit lasts up to an hour and focuses on health history, medications, and risk factors to close care gaps.

Qualifications
- Master’s degree or commensurate experience with NP licensure.
- Current NP licensure in state of practice.
- Board certification in Adult, Gerontology or Family specialties.
- BLS/ACLS/CPR certification.
- Valid driver’s license and travel willingness.
Responsibilities
- Conduct adult/geriatric assessments including history, diagnosis, treatment, and documentation.
- Administer and order point-of-care and lab-based testing as needed.
- Collaborate with physicians, case managers and other care team members.
- Provide services in patients' homes or Skilled Nursing Facilities.
