Matrix Medical Network seeks a Licensed NP to perform comprehensive in-home health assessments in Lincolnville, ME, with regular travel to the surrounding area. The role involves reviewing medical history, medication adherence, and risk factors to close care gaps.

Responsibilities include adult/geriatric assessments, ordering tests, and collaborating with PCPs. Sign-on bonus and flexible scheduling enhance the opportunity.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure with prescriptive authority or ability to obtain.
• AANP, AACN, or ANCC board certification in adult/gerontology/family/acute care.
• BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license for the state of residence.
• Auto insurance coverage meeting state requirements; Bodily Injury $100k/pp, $300k/incident; Property Damage $50k; Collision deductible $1,000 or less.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.

Responsabilités

• Conduct adult / geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other clinicians.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence (home or Skilled Nursing Facility).
