Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive health assessments in patients’ homes or Skilled Nursing Facilities. Responsibilities include reviewing medical history, diagnoses, and medications, ordering tests, and coordinating care with physicians and case workers.

Travel up to 80–90% within the assigned state may be required. Ideal candidates hold an NP certification with prescriptive authority, current NP licensure, and strong communication skills.

Qualifications

• Master’s NP degree and licensure in state of practice.
• Board certification in relevant NP specialty required.
• Current NP licensure with prescriptive authority; ability to obtain as needed.

Responsibilities

• Conduct adult/geriatric health assessments including history, diagnosis, education, and referrals.
• Administer/order point-of-care and lab testing as needed.
• Coordinate with physicians, case managers, and care teams, and document per Matrix protocols.
• Collaborate with PCP on patient education and follow-up.
• Provide in-home or SNF-based assessments at members’ residences.
