Job summary

Matrix Medical Network seeks a skilled Nurse Practitioner to perform Comprehensive Health Assessments in members’ homes or SNFs, with visits up to one hour and travel to Russell, PA as needed.

You will evaluate medical history, medications, and risks, conduct tests, and coordinate with PCPs and care teams to close care gaps. Flexible scheduling and ongoing clinical education support are provided.

Qualifications

• Must hold NP licensure in practicing state and prescriptive authority if needed.
• Master’s degree or commensurate experience with NP licensure.
• Board certification in Adult/Adult Gerontology/Family/Acute Care/ Emergency NP

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers, social workers, family and other care team members.
• Provide services at member's residence (home or SNF).
• Document findings clearly and coordinate follow-up care.
