Job summary

Sweetwater Care in Butte, MT is seeking an LPN/LVN to provide nursing care within scope of practice in accordance with standards and orders. The ideal candidate has LTC experience and is familiar with the Point Click Care charting system.

Responsibilities include charting and incident reporting, narcotic counts, and documentation of patient condition changes. BLS/CPR certification is required or willingness to obtain; nursing management experience is a plus but not required.

Qualifications

Responsibilities

• Follows established charting procedures and incident reporting.
• Manages narcotic counting, pharmacy orders, and restraint reduction per procedures.
• Documents changes in patient conditions (nutrition, infections, weight, skin, etc.).
• Uses Quality Life Manual to identify, place, and monitor patients in programs.
• Notifies departments of room changes, LOA, new admits or discharges.
• Makes rounds with physicians when necessary.

Provides nursing care within scope of practice to ensure patient's needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
