Job summary

Sweetwater Care is seeking an LVN/LPN to provide skilled nursing care within scope of practice, following physician orders and center policies in a long-term care setting.

The ideal candidate holds an active LVN/LPN license in Montana, with BLS/CPR certification or willingness to obtain it, and has prior long-term care exposure. Experience with PointClickCare charting is preferred, and management experience is a plus.

Qualifications

• Current LVN/LPN license in practicing state.
• BLS/CPR certification or willingness to acquire one.
• Long Term Care experience.
• Point click care charting experience.
• Nursing management experience preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including nutritional problems, infections, skin conditions, weight loss or other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
