Job summary

Sweetwater Care in Montana is seeking a licensed practical nurse (LPN/LVN) to provide patient care within scope of practice, adhering to physician orders, center policies, and state guidelines. You will perform accurate charting, respond to changes in condition, and collaborate with physicians during rounds.

The ideal candidate has long-term care experience, an active LVN/LPN license, and current BLS/CPR certification or willingness to obtain one.

Qualifications

• Long Term Care experience.
• Point click care charting experience.
• Active LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Nursing management experience is preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
