Job summary

Sweetwater Care in Montana is seeking an licensed practical nurse (LPN/LVN) to provide professional nursing care within scope of practice and in accordance with physician orders and center policies. The role ensures residents' needs are met while complying with state and federal guidelines.

Responsibilities include charting, incident reporting, narcotics counting, and providing updates on patient conditions. Rounds with physicians may be required.

Qualifications

• Long Term Care experience required.
• PointClickCare charting experience required.
• Active LVN/LPN license in practicing state.
• BLS/CPR certification required or willingness to obtain.
• Nursing management experience preferred but not required.

Responsibilities

• Follows established charting and incident reporting procedures.
• Adheres to narcotic counting, pharmacy orders and restraint reduction protocols.
• Documents changes in patient conditions (nutrition, infections, weight loss, etc.).
• Uses Quality Life manual for patient programming identification and monitoring.
• Notifies relevant departments of room changes, LOA, admits, or discharges.
• Participates in rounds with physicians when necessary.

POSITION DESCRIPTION

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
