Job summary

Sweetwater Care is seeking a licensed LVN/LPN to provide nursing care within scope of practice, adhering to standards of practice, physician orders, and center policies. The role emphasizes patient safety, careful documentation, and collaboration with the clinical team.

The ideal candidate will have long-term care experience, familiarity with PointClickCare charting, and an active LVN/LPN license in the practicing state. BLS/CPR certification is required or must be obtained.

Qualifications

• Must have long term care experience.
• Experience with PointClickCare charting.
• Current LVN/LPN license in the practicing state.
• Current BLS/CPR certification or willingness to obtain.
• Nursing management experience is preferred but not required.

Responsibilities

• Follows procedures for charting and incident reporting.
• Follows procedures for narcotics counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions (nutrition, infections, skin conditions, weight loss, etc.).
• Uses the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notes room changes, LOA, new admits, or discharges to applicable departments.
• Makes rounds with physicians when necessary.

QUALIFICATIONS

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
