Job summary

Sweetwater Care is seeking an experienced LPN/LVN to provide nursing care in a long-term care setting. You will chart using PointClickCare, monitor patient conditions, and follow established policies while collaborating with the care team.

The role requires an active LVN/LPN license, BLS/CPR certification, and a willingness to obtain further credentials if needed. Nursing management experience is a plus but not required.

Qualifications

• Must have active LVN/LPN license in the practicing state.
• Experience in long-term care settings.
• Experience with PointClickCare charting.
• BLS/CPR certification or willingness to obtain.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions including nutrition, infections, skin conditions, weight loss, or other changes.
• Uses Quality Life Manual for patient identification, placement, and monitoring in programs.
• Documents and notifies departments of room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
