Job summary

Sweetwater Care seeks a licensed LVN/LPN to provide nursing care within scope of practice, following physician orders and center policies in California.

The role requires long-term care experience, PCC charting, and current BLS/CPR certification. Nursing management experience is a plus, with opportunities to observe and collaborate with physicians during rounds as needed. Based in Porterville, CA, the facility offers a supportive team and ongoing training.

Qualifications

• Long term care experience required.
• Familiarity with Point Click Care charting.
• Current LVN/LPN license in California.
• Current BLS/CPR certification or willingness to acquire.
• Nursing management experience preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions including nutrition, infections, skin issues, weight changes.
• Uses Quality Life Manual for patient identification, placement, and monitoring.
• Documents and notifies departments of room changes, LOA, admits, or discharges.
• Makes rounds with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• LONG TERM CARE EXPERIENCE
• POINT CLICK CARE CHARTING EXPERIENCE
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
