Job summary

Hospice of Southern Illinois, Inc. is seeking a Staff Registered Nurse/Case Manager to assess patient needs, plan and implement care, and coordinate with the IDT. The role involves driving within the Marion eastern territory and a Monday–Friday schedule with on-call rotation.

Based out of the Marion Patient Services Office, travel is extensive, with reimbursement at .70 per mile. Licensure in Illinois and hospice experience are preferred; benefits include PTO, health/dental/vision, disability,

Qualifications

• Graduate of State-approved nursing program.
• Associate Degree in Nursing or Diploma in Nursing required.
• One year hospice experience preferred.
• Current Illinois nursing license.
• Certification in hospice and palliative care preferred.
• CPR certification preferred.
• Excellent leadership, managerial ability, clinical assessment, organizational and interpersonal relationship skills.
• Knowledge of Medicare/Medicaid, hospice regulations, and third-party payor expectations.
• Reliable transportation.

Responsibilities

• Maintain a caseload of patients and coordinate care with the IDT.
• Provide direct patient care using the nursing process.
• Complete annual skills and clinical assessments during visits.
• On each visit, perform a physical assessment, medication review, and symptom evaluation; communicate findings and obtain orders.
• Communicate pertinent information to all disciplines via phone, text, or IDT meetings.
• Review and update the Plan of Care on each visit.
• Participate in on-call after-hours care as needed.
• Document assessments timely per agency policies and standards.
