Job summary

Hospice of Southern Illinois is hiring a Staff Registered Nurse/Case Manager based in Belleville, IL. You will assess patient needs, plan and implement care, and coordinate with the IDT to ensure quality end-of-life support in home or home-like settings.

The role requires an Illinois nursing license, hospice experience preferred, and a valid driver’s license for travel within service areas. Benefits include PTO, health/dental/vision, life and disability insurance, and a 403(b) retirement plan.

Qualifications

• Graduate of a state-approved school of nursing.
• Current Illinois nursing license is required.
• One year hospice experience preferred.

Responsibilities

• Maintain a caseload of patients and coordinate care with the IDT.
• Provide direct patient care, including on-call after hours care.
• Update and communicate the Plan of Care on each visit.
• Collaborate with IDT to meet patient and family needs.
