The Care Team Hospice in Springfield, IL is seeking a compassionate Hospice RN Case Manager to coordinate patient care and lead individualized plans of care, collaborating with physicians, caregivers, and the interdisciplinary team to ensure comfort and dignity at end of life.

This role offers a Monday–Friday schedule with on-call opportunities and a supportive, growth-focused environment, plus a comprehensive benefits package including medical, dental, vision insurance and paid time off.

Formación

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsabilidades

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.

Qualifications:

• Active RN License required
• Hospice nursing experience preferred
• Experience with Homecare Homebase (HCHB) preferred

Schedule & Work-Life Balance

• Monday–Friday, 8:00 AM – 4:30 PM
• On-call opportunities available with additional compensation
• Some locations offer dedicated on-call teams, resulting in backup coverage only

Key Responsibilities:
