Hospice of Southern Illinois is seeking a dedicated Staff Registered Nurse/Case Manager based in Belleville, IL. You will assess patients, plan and coordinate care, and work with the interdisciplinary team to meet end-of-life goals.

The role requires an Illinois nursing license, hospice experience preferred, and a valid driver’s license with reliable transportation for home visits within the service area.

Qualifications

• Graduate of a state-approved nursing program.
• Current Illinois nursing license.
• One year hospice experience preferred.

Responsibilities

• Maintain a caseload of patients and coordinate caregiving with the IDT.
• Provide direct patient care and participate in after-hours on-call as needed.
• Document assessments, plan of care updates, and medication orders.
