Job summary

Sage GroupHealthCare Staffing seeks a Hospice Registered Nurse Case Manager for a 13-week contract in Davenport, IA. This full-time travel/contract role covers Home Health Hospice with a 40-hour work week and a travel/housing stipend for eligible travelers.

Start ASAP, requires an RN license, hospice/home health experience, and strong charting skills (HCHB). Mileage reimbursement is provided at $0.49/mile. Applies to both local and traveler candidates.

Qualifications

• Registered Nurse with 1 year hospice or home health experience
• Current RN license for Iowa or compact state
• BLS certification from AHA or Red Cross is required
• Must have valid driver's license and auto insurance
• Covid vaccination or exemption acceptable

Responsibilities

• Provide and coordinate hospice care in patients' homes
• Develop and implement Start of Care and admission nursing plans
• Educate patients and families on disease management and care goals
• Document all visits within 24 hours and maintain charting
• Cover Davenport to Muscatine territories as assigned
