Job summary

Sage GroupHealthCare Staffing seeks a Hospice RN Case Manager for a travel/contract position in Blue Ash, OH. Accepting both local and traveler candidates. This role focuses on comprehensive hospice care coordination in home settings across the Cincinnati area, with a fixed 40-hour workweek and on-call weekends.

The nurse will develop care plans, educate families, and ensure compliance with state regulations while managing a defined caseload and utilizing HCHB charting to document patient status.

Qualifications

• Registered Nurse: 2 years - Required
• Recent Hospice nursing experience in a home health setting: 1 year within the last 3 years – Required
• Home Care Home Base(HCHB) charting experience – Required
• Must have valid Driver’s License and Personal Auto Insurance
• Must be Covid Vaccinated or provide medical or religious exemption

Responsibilities

• Provide and coordinate care for assigned Hospice patients and manage assigned caseload
• Develop and implement nursing care plans for patients as assigned
• Educate patients and families to reach healthcare goals
• Provide Hospice nursing care within state regulations and policies
• Ensure physicians’ orders are legible and transcribed correctly
• Document and complete charting and status updates
• Territories: Greater Cincinnati area Mon-Fri; Cincinnati and Dayton on-call weekends
