Job summary

Sage GroupHealthCare Staffing is seeking a Hospice Registered Nurse Case Manager for a 13-week travel/contract position in Montrose, CO. The role covers home and community settings with 40 hours/week and on-call rotation.

You will manage a caseload, provide assessments, care planning, pain and symptom management, and ensure timely charting in line with client policies. Valid RN license and BLS are required.

Qualifications

• RN with 1–2 years hospice experience.
• EMR/EHR charting is required.
• ASN or equivalent diploma is required.
• BSN is preferred.
• RN license in the applicable state or a compact license is required.
• BLS certification required.

Responsibilities

• Provide nursing care for hospice patients in homes or facilities.
• Manage assigned caseload and lead the interdisciplinary team.
• Perform assessments, care planning, pain and symptom management.
• Ensure timely charting in line with client policies.
• Cover Montrose, CO and surrounding areas.
