Sage GroupHealthCare Staffing seeks a Hospice Registered Nurse Case Manager for a 13-week contract in Davenport, IA. This full-time travel/contract role covers Home Health Hospice with a 40-hour work week and a travel/housing stipend for eligible travelers.

Start ASAP, requires an RN license, hospice/home health experience, and strong charting skills (HCHB). Mileage reimbursement is provided at $0.49/mile. Applies to both local and traveler candidates.

Competenze

• Registered Nurse with 1 year hospice or home health experience
• Current RN license for Iowa or compact state
• BLS certification from AHA or Red Cross is required

Job Description

• Provide andcoordinate care for assigned hospice patients in comfort of their homes.
• Develop and implementnursing care plans including Start of Care and admissions as assigned.
• Support patientrecovery and ability to stay at home - enabling them to spend quality timewith their loved ones and doing the activities they enjoy.
• Empower patients andfamilies to reach healthcare goals by educating them on disease management.
• Provide Hospice nursingcare and services within the standards and scope of nursing practice asdefined within the state Nurse Practice Act regulations, policies, procedures,and protocols, as well as facility and corporate policies and procedures.
• Ensure physicians’orders are legible if taken verbally and transcribed appropriately perpolicy and procedure.
• Ensure patients’status is assessed on an ongoing basis and pertinent information isgathered and documented.
• Notify physicians ofsignificant changes in status.
• Document and completecharting for all patient visits within 24 hours.
• Provide bereavementsupport to patient’s families as needed.
