The role requires RN licensure, ASN (BSN preferred), and 2+ years hospice/home care experience, plus valid driver’s license and Covid vaccination or exemption.

Responsibilities

• Provide and coordinate hospice care in patients' homes.
• Develop and implement Start of Care and admissions nursing plans.
• Case manage and oversee interdisciplinary teams.
• Document care and chart within 24 hours of visits.
• Provide Bereavement support to families.
