Sage GroupHealthCare Staffing is seeking a Hospice Registered Nurse Case Manager for a 13-week contract in Lacey, WA. The role focuses on coordinating hospice care at patients' homes, developing care plans, and ensuring timely visits and charting.

The ideal candidate has 1 year of hospice or home health experience, an ASN, and is licensed in the state or compact. A BSN is highly preferred and a valid driver’s license is required.

Qualifications

• 1 year of RN experience in home health or hospice required
• Home health or hospice home-based experience within last 3 years required
• HCHB charting experience required
• Valid driver’s license and personal auto insurance required

Responsibilities

• Provide and coordinate care and case management for hospice patients in their homes
• Develop and implement nursing care plans including SOC and visits
• Perform case management and various home visits for assigned caseload
• Support patient comfort and pain management to enable staying at home
• Educate patients and families on disease and pain management
• Ensure documentation and charting within 24 hours of visits
• Provide bereavement support to families as needed
• Territories to cover: King County (4–5 visits/day)
