Job summary

Sage GroupHealthCare Staffing seeks an RN Hospice Case Manager for a 13-week travel/contract position in Grand Junction, CO. The role accepts local and traveler candidates and requires 40 hours/week with a 5-day workweek, Mon-Fri, with occasional weekend/on-call backup.

Responsibilities include managing a caseload, coordinating interdisciplinary care, and ensuring timely documentation. ASN required; BSN preferred; RN license and BLS are mandatory.

Qualifications

• EMR/EHR charting is required.
• ASN or equivalent; BSN preferred.
• RN license for state applied or compact.
• BLS certification required.

Responsibilities

• Provide nursing care for hospice patients in private homes, nursing homes, group homes or assisted living facilities.
• Manage assigned caseload of patients and lead the interdisciplinary team caring for those patients.
• Provide assessments, care planning, pain management and symptom management in hospice nursing.
• Ensure timely and accurate submission of patient documentation and charting per client policies.
• Territories to cover: Grand Junction, CO and surrounding areas.
