Job summary

Fresenius Medical Care is seeking a Home Therapies RN CAP 2 to provide and coordinate clinically competent care for an assigned group of patients requiring dialysis therapies. This role involves collaboration with RNs, LVN/LPNs, and Patient Care Technicians, and may include travel to patient homes and training sites.

The RN CAP 2 serves as a resource and mentor, participates in education and quality improvement activities, and works within policy, procedures, and regulatory standards to ensure

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful completion of CPR BLS Certification.
• Entry level RN with minimum 2 years or more of Nephrology Nursing experience within the last 2 years.

Responsibilities

• Provide and coordinate clinically competent care including assessment, planning, intervention and evaluation.
• Collaborate with the interdisciplinary team to deliver dialysis therapies and patient education.
• Direct and mentor care staff to ensure safe, effective delivery of home dialysis services.
• Participate in CAP requirements and professional development as assigned by supervision.
