Job summary

Fresenius Medical Care is seeking an RN CAP 2 for Home Therapies to provide and coordinate dialysis care for an assigned patient group, including planning, interventions, and evaluation. The role involves collaboration with RNs, LVNs/LVNs, and Patient Care Technicians, training home dialysis patients, and contributing to quality improvement and CAP progression.

Graduate of accredited nursing program with active state licensure and CPR/BLS; prior nephrology nursing experience preferred; on-site

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• CPR BLS Certification Must meet the practice requirements.

Responsibilities

• Provide and coordinate dialysis patient care per policy.
• Collect and analyze dialysis data; adjust treatment plans as needed.
• Educate patients and families on care plans and home dialysis options.
• Serve as a leader and mentor to staff; participate in quality improvement.
