Job summary

Fresenius Medical Care in Utah is seeking a Home Therapies RN CAP 2 to provide and coordinate dialysis care for an assigned group of patients, including education and collaboration with the interdisciplinary team. This role may involve supervision and delegation as part of the CAP program.

The RN will support home dialysis patients, participate in on‑call rotations, and may travel regionally for training and coverage across sites.

Qualifications

• Graduate of accredited School of Nursing.
• Current state RN licensure.
• CPR/BLS certification.

Responsibilities

• Provide and coordinate clinically competent care including assessment, planning, intervention and evaluation.
• Educate patients and families on treatment plans.
• Direct home dialysis treatments and training with Home Partners.
• Participate in facility quality improvement activities.
